AAAHC leads accreditation for ambulatory health care through nationally recognized quality improvement standards and a survey experience founded on a peer-based, educational approach to onsite review. The AAAHC Mission: To encourage and assist ambulatory health care organizations to provide the highest achievable level of care for recipients in the most efficient and economically sound manner.

Referrals & Specialist Coordination
When your healthcare needs extend beyond our services, we ensure seamless transitions by coordinating referrals to trusted specialists. Whether you require care from a cardiologist, orthopedist, or another specialist, we work closely with them to manage and monitor your overall health. We stay involved every step of the way, making sure your care is continuous, comprehensive, and tailored to your needs.

Ronnie Diem, PA-C
Physician Assistant
I’m Ronnie, a certified Physician Assistant here at Coastal Medical. I grew up in Dover and graduated from Dover High School. I’m also a proud US Army Veteran, having served as an Airborne Medic in a Combat Arms Unit.
At Coastal Medical, I provide essential medical care, including diagnosing illnesses, developing treatment plans, and offering a wide range of healthcare services to our patients. I work closely with the medical team to manage both acute and chronic conditions, perform physical exams, and assist with minor procedures, ensuring that every patient receives comprehensive and compassionate care.
When I am not at work, I enjoy coaching lacrosse, going to the beach, and spending time with my family and friends.
Gabrielle Howlett, PA-C
Physician Assistant
I’m Gabrielle, a certified Physician Assistant. I graduated from Pfeiffer University in North Carolina from the Physician Assistant program in May 2024. I’m passionate about helping patients and providing them with the resources they need to get better.
When I’m not at work, I love spending time with my dog, who I couldn’t imagine living without. If I could travel anywhere in the world, I’d love to go to Thailand and participate in an elephant rescue. My favorite treat is Reese’s—definitely my go-to snack!
Fidel Romo-Martinez
MSN, APRN, FNP-C
Born and Raised in Southern California, I received a football scholarship to play Division 1 Football at Delaware State University. I graduated Valedictorian of the graduating class in 2019 with a Bachelors of Science in Biology and a minor in Chemistry. In 2021, I completed Thomas Jefferson University’s FACT-1 accelerated Nursing program. I worked as a registered nurse at a Level 1 Trauma hospital known as Thomas Jefferson University Hospital for three years in an intermediate Neurology Intensive Care/ Spine Orthopedics Unit. While working as a registered nurse, I enrolled in Thomas Jefferson’s Family Nurse Practitioner program and graduated in May 2024. While in the program, I developed a strong interest in Outpatient Family and Acute Care Medicine. As a Nurse Practitioner, I am dedicated to providing high-quality care to my patients. With a strong educational background and a commitment to ongoing learning, I strive to stay at the forefront of medical advancements in order to offer the best possible care. As a prior bedside nurse, I have seen patients at the end of life and during their moments of wishing they had listened to prior medical advice and I’m here to partner with you to aid in the prevention of chronic disease development or progression while utilizing the most up to date evidence-based practice.
During my free time I enjoy spending time with my wife biking at different parks, traveling, and exercising.
Katelyn Curtis
Medical Assistant
I’m Katelyn and I am a medical assistant here at the clinic. When you visit our walk-in, I’ll be the one greeting you with a smile and assisting you throughout your entire visit. With over six years of experience, I take pride in providing knowledgeable, compassionate care. Family is a priority for me, and I aim to offer that same level of care and attention to every patient I meet, making sure you feel comfortable and at home during your time with us.
Outside of work, I love spending time with my family, playing with my two dogs, Bleu and Jaxe, and unwinding with a good book.
Kirby Brown
Medical Assistant
I’m Kirby and I was born and raised in Delaware. I began my medical assisting career in 2017 after graduating from Harris School of Business. I have a passion for helping others, and I always strive to go above and beyond to ensure my patients are well taken care of. On the primary care side, I work hard to keep things organized and running smoothly. With over seven years of experience as a medical assistant, I’m confident you’ll be in great hands. I’m here to help facilitate your needs, whether it’s scheduling appointments, handling medication refills, or making sure your questions are answered.
When I’m not in the office, I love spending time with my husband and two children. We enjoy going to the beach and are always looking for new family activities to try!
Desi
Allen
Medical Assistant/Receptionist
I’m Desi, a medical assistant and front desk receptionist here at the clinic. I graduated from the Harris School of Business as a medical assistant and have been in the medical field for 14 years, starting in home health as a teenager. Before joining the team here, I worked at Camden Medical Center since 2015 and also spent time with Bayhealth.
What I love most about working at the clinic is the incredible staff and patients we care for every day. The positive and supportive environment that Ronnie and Amy have created makes it a great place to work, and I really appreciate how we come together as a team to get things done.
Outside of work, I enjoy being outdoors in nature and expressing myself through art. I’m also a true Maryland girl—I love crabs, and they’re my favorite food! My kids are my world and my “why”—I couldn’t imagine life without them.
Penny
Clark
Medical Assistant/Receptionist
I’m Penny and I graduated from Polytech Adult Education and hold a certification in phlebotomy. Since starting my medical career in 2009 in home health, I’ve earned my CNA certification in 2012, became an ER tech in 2017, and began working as a medical assistant in 2021. I love helping people, providing assistance, and ensuring great care for all who walk through our doors.
My passion for healthcare comes from a personal experience. I chose this path after my child was diagnosed with cancer at just four months old and, unfortunately, passed away at the age of two. The incredible healthcare we received during that difficult time inspired me to give back. My goal is to provide the same high-quality care to others when they need it most.
I love my patients, my team, and the impact we can make on others’ lives. I always strive to make a difference by offering compassionate care, maintaining great bedside manner, and treating everyone with the kindness they deserve.
Outside of work, I love cooking—lasagna is one of my favorites to make! I’m also passionate about music and couldn’t live without it. In my free time, I enjoy playing pool, riding motorcycles, traveling, and spending time with my kids.
Amy Diem
Office Manager
I’m Amy. Although I grew up in Delaware, I’ve had the chance to live in different parts of the country. I’ve always had a passion for helping others, which led me to pursue a degree in education from Lee University in Tennessee. I continued my studies there, earning an add-on degree in special education, and later completed my master’s degree in English as a Second Language from Salisbury University. While I’m an educator by trade, I’ve also worked in various roles within the community, all focused on supporting people.
At Coastal Medical Center, I’m committed to helping our incredible staff by providing them with the support they need and fostering a positive environment that encourages them to thrive. I truly believe the Coastal team is amazing, and they make my job a joy every day.
When I am not at the office, I enjoy the beach and spending time with my family and friends.
Harold Spangler, MD
Medical Director
I’m Dr. Harold Spangler, and I serve as the Medical Director of Coastal Medical. I oversee our clinical operations to ensure we’re delivering high-quality medical services to our patients. With a strong background in patient care, I provide leadership to our medical team while also offering direct care to patients. My goal is to ensure that Coastal Medical stays at the forefront of healthcare, providing both urgent care and primary care services that our community can rely on.
